During cataract surgery, the cloudy natural lens is removed and replaced with a clear artificial intraocular lens (IOL). This IOL sits inside the eye's natural lens capsule , a thin, transparent membrane that holds it in place.
In 20–50% of patients, this capsule gradually becomes cloudy or wrinkled over time , a condition called posterior capsule opacification (PCO). This causes vision to become hazy, blurry, or glared, similar to how it felt before cataract surgery. It is sometimes called a “secondary cataract,” though it is not a new cataract forming.
YAG laser capsulotomy uses a focused laser beam to create a small, precise opening in the clouded capsule. This instantly allows light to pass through clearly to the retina, restoring sharp vision. The procedure is performed in clinic, takes about 5 minutes, and requires no incisions or stitches.

The Procedure
Assessment
Your consultant examines your eye, confirms posterior capsule opacification (PCO) is the cause of your cloudy vision, and checks that YAG laser treatment is appropriate.
Dilating Drops
Drops are applied to dilate your pupil and numb your eye. No injections or needles are used. You will be comfortable, with only mild discomfort expected.
Laser Treatment
A contact lens is placed on your eye to guide the laser. Precise pulses of YAG laser energy create a small opening in the clouded capsule behind your artificial lens, allowing light to pass through clearly.
Pressure Check
Your eye pressure is checked approximately 30–60 minutes after the procedure to ensure it is stable. Anti-inflammatory drops may be prescribed for a few days.
Recovery
You go home the same day. Most patients notice clearer vision within 24–48 hours. You cannot drive immediately after, so arrange transport. Normal activities resume the next day.

PCO occurs when the thin membrane (capsule) that holds your artificial lens in place after cataract surgery becomes cloudy or wrinkled. This is sometimes called a 'secondary cataract,' though it is not a new cataract. It affects 20–50% of cataract surgery patients within 2–5 years and causes gradually worsening blurry or hazy vision.
The eye is completely numbed with anaesthetic drops. You may see bright flashes of light during the laser treatment and feel the contact lens on your eye. Most patients experience only mild discomfort, if any. The entire procedure takes approximately 5 minutes.
Most patients notice significantly clearer vision within 24–48 hours. Some experience improvement within hours. You may notice floaters for a few days to weeks after the procedure , these settle naturally as debris from the capsule opening is absorbed.
Once the cloudy capsule has been opened by the YAG laser, it does not regrow or become cloudy again in the same area. For most patients, a single YAG treatment provides permanently clear vision.
YAG laser capsulotomy is very safe. The most common side effects are temporary floaters and a transient rise in eye pressure (monitored in clinic). Retinal detachment is a rare risk (approximately 0.2–0.5% within the first year). Your consultant will explain all risks before proceeding.
